Physician Extenders on Episode 8 of Insights in Nursing

This week's episode of Insights in Nursing looked at a USA Today article on multi-state licenses causing problems with disciplined nurses skirting the rules. Is this a huge problem or much ado about nothing?

Then we move on to discuss a problem with the title some physician agencies are using for PAs and Nurse Practitioners. They are being called "Physician Extenders." Huh? EXACTLY.

Finally we point out an instance of the media getting it right by recognizing a nurse who was nominated to a position of honor by a patient advocacy group.

This is Episode 7 of Insights in Nursing.

This week's episode of Insights in Nursing kicked off with a great discussion about critical understanding of research published by various sources. The topic came from a great article about flawed meta-analysis in a research study by Amy Romano of the Science and Sensibility blog (ScienceandSensibility.org).

We followed that up with a look at EMS blogger, Chris Kaiser's article on EMTs and Paramedics as "Sporks" of the medical community, in which he referred to nurses as the perfect and natural assistants to doctors. Chris wasn't being malicious but was repeating a common misconception about nurses that seems to continue to exist despite all that nursing has accomplished over the years.

What is it about blogging that scares employers so much and what can we do to ensure that health care bloggers act responsibly when they discuss medical and nursing topics?

Tip of The Week- Pharmacokinetics, Pharmacodynamics and Nursing Care

Pharmacokinetics is defined as the process by which a drug is absorbed, distributed, metabolized, and eliminated by the body. In other words, it is the study of how the drug moves into the body from the point of administration as it goes into the different parts of the body then broken down and later on released via the involved organs involved in the excretory system.

Pharmacodynamics on the other hand is defined as the study of the biochemical and physiological effects of drugs and the mechanisms of their actions, including the correlation of their actions and effects with their chemical structure. It is the study of the effects of substances or drugs to the body.

As nurses, it is important to have knowledge of the pharmacodynamics and pharmacokinetics of drugs to better understand how they work in the body and how they affect physiological processes. This knowledge can be applied in assessing our patients on their response to medications and to better explain to patients how their medications work and why it is important to adhere to them.

An article from Emergency Medicine News about potential new rules from the CMS. This time, I actually think it’s a good idea. They’re looking at benchmarking throughput times for ED patients. This may give hospital administrations a financial incentive to get admitted patients OUT of the ED sooner! Let’s hope!

Also, an article from EMS Responder online magazine about patient assessment. This is where the entire continuum of care truly begins and it needs to be correct. I love this article. Take a look and see what you think!

Another article from Emergency Medicine News, this time on Propofol and the new ruling from the FDA on its use in areas outside the OR. Very interesting article!

A “fun” article from JEMS.com that looks back on some history in emergency medicine and the tools we used to use. For those who have been around as long as I have or longer, back from when EMS was truly in its’ infancy, it’s a fun trip through memory lane. And for those who are relatively newer, it might be cool to read about some of the things we used to use.

An article form Medical News Today about the questionable benefits of tight blood pressure control.

It’s a diabetes heavy news spot this week.
First up, an article from WebMD that diabetes doubles a person’s risk of heart attack and stroke.

A second article from WebMD on a new class of diabetes medicine. This one will help control a person’s sugar AND help control their weight. Very interesting!

Another article on diabetes from WebMD. This one on a new way to administer insulin…..inhaled! This promises to help people stay more compliant with their medicines!

And from the “I can’t believe they did that!” files….a hospital used an off duty police officer to pretend to be a terrorist who stormed the ICU and took the nurses “hostage”. He then told them it was a drill. This could have been a MAJOR problem for lot of reasons. Read the article from the Las Vegas Sun to learn more.

We wrap up this portion of the episode with a look at Samson University's huge nursing program in Alabama. Terri and Kim note that all of the graduates in the photo of the recent pinning ceremony are wearing their caps.

I’m away in Pa. this week, but I managed to get the show ready to go before leaving. But since I’m away, there will be minimal blog updates until Friday morning after I’m back in Fl.

An update to the PhoenixCE site is still in the works. With the recent death of my father, I’ve had minimal time to put toward the blog, the show, or the other projects I have going here. Once I get back from Pa I should have more time to devote to them though. Look for updates to roll out this summer!

Along with that, I put up a new poll. Tell me what you think about taking a FREE ESI course online LIVE. It would be for con ed credit also.

I also have an ABG interpretation class and EKG interpretation classes in the works. I will be announcing dates for them soon too. CE credit will be available for them too, so look for them!

In the news today:

A story from Cult of Mac about a hospital purchasing 100 iPads to give to physicians and nursing staff as a trial program. As I blogged about my own iPad on Sunday, I can definitely see it coming to the bedside in full force in the very near future.

An editorial from the Ashland Daily Tidings out of Oregon titled “Too much medicine”. A very good, thought provoking piece about how we practice medicine in the US.

An article from Medline Plus that goes right along with the last piece on the cost of imaging services for cancer patients. It’s interesting that we are doing more imaging on patients who already have cancer, and the tests we do are actually linked to an increase in cancer rates. Makes one wonder.

A JEMS article on the potential negative effects that placing cervical collars can have on patients neurovascular status. Again, a thought provoking article on something we do routinely that has never been shown to be the “best practice”.

Another JEMS article, this time that covers traumatic brain injury. This is a really good primer/overview/review on a subject that we need to stay on top of.

An article out of Medline Plus that discusses research into what interruptions of nurses during medication rounds can do. Med errors are a big potential problem in health care, and interruptions during their administration are one reason for these errors. This article discusses the topic and offers some interesting ideas on how to minimize these distractions.

Tip of The Week- K2 and Designer Drugs with Toxicology Specialist Lisa Booze

K2, Fake weed, spice, and zohai are just some of the names given to synthetic marijuana. It is a chemical that is intended to substitute marijuana and mimics its effects only more potent. It may be a mixture of herbal and spice plant products, but it is sprayed with a potent psychotropic drug and likely contaminated with an unknown toxic substance that is causing many adverse effects. Unlike marijuana, reactions from using K2 include agitation, anxiety, tachycardia, and elevated blood pressure.

There are also synthetic/designer drugs out there like K2 that are contaminated with unknown substances and untested substances making it difficult to identify and predict its effects. Nurses should continuously assess these patients for unexpected adverse reactions and document their findings. Coordination with poison control centers and providing them the information gathered from these patients will help in providing effective nursing interventions and treatment and also to have reference for same cases that may be encountered in the future.

Tip of The Week- Review of Methicillin Resistant Staphylococcus Aureus

MRSA, otherwise known as methicillin-resistant staphylococcus aureus, is a type of staphylococcus bacteria that is resistant to certain types of antibiotics. It can either be hospital associated (HA-MRSA) which happens in the healthcare setting or community acquired (CA- MRSA) which is passed through skin contact with infected people.

MRSA may present as a skin infection or a bump which may be red, swollen, painful, warm to touch, with pus or drainage and accompanied by fever.

The incidence of MRSA in both hospital and community setting lies in proper infection control and good hand hygiene. Some of the things that nurses should remember when it comes to the prevention of the spread of MRSA in the healthcare setting includes, wound and drainage precautions, disinfection of articles contaminated with wound drainage and practice of proper handwashing in between patients.

January 27th was the public debut of the Apple iPad, and what a debut it was! I don’t know how many of you follow Apple and their products, but let me tell you………this little beauty is going to be a game changer in so many ways! one thing I kept thinking of as I was watching the video of the press event was how it could impact health care. Imagine having something about the size of a clipboard chart that gives you real time updates of labs and other information, lets you add care note, do your assessment, etc., can let you see and show the patient their x-rays, and even check the status of other patient’s — all right at the bedside — wirelessly!!! Look for this type of device to come to bedside patient care sooner than later…….I’d bet anybody on that.

And coming on the heals of that, I plan to review a few nursing/medical/patient care related apps on the next show. Have any that YOU like to use? let me know what they are and I may include them in the show!

Also, work continues on offering live, online courses for things like ACLS prep, PALS prep, etc. Look for more definitive information within the next month or so.
