In this episode we talk with Alameda County AMR General Manager, Mike Taigman about an innovative approach to managing asthmatic patients in Alameda County with a proactive approach to identifying patients at risk for exacerbation of chronic asthma. Using a simple assessment and education tool, paramedics and EMTs in Alameda County can help patients get a handle on their condition. Certain patients will be followed with a visit by a team of paramedics and EMTs called a Self Care Team. |
